Follow these steps below to access your employer’s doula benefit:

  1. Reach out to your employer’s human resources department and see if they have a doula benefit and clarify the amount and what paperwork needs to be completed.
  2. Reach out to Chicago Family Doulas via email or contact form.

3. Arrange a call with our family coordinator and we will begin the matching process.

4. We will match you with a certified birth or postpartum doula and they will complete all needed paperwork and submit their certification and proof of insurance to you.

5. You will submit all documents including the completed form, proof of certification and proof of insurance to your employer.

6. We will send you detailed invoices including all the information your employer requires.

7. You pay the invoices and submit to your employer for reimbursement.
